Intensive In-Home (IIH) — Referral Assessment

Service Description

WISE Enterprise Inc. provides Intensive In-Home (IIH) services across Virginia, authorized under the Virginia Department of Medical Assistance Services (DMAS) and the Department of Behavioral Health and Developmental Services (DBHDS). IIH delivers family-centered, community-based behavioral health support for youth (up to age 21) with a mental health or behavioral diagnosis, provided in the home and natural community settings. SERVICES INCLUDE: • Individual and family therapy in the home • Behavioral intervention and crisis stabilization • Parent/caregiver training and psychoeducation • Safety planning and crisis prevention • Community integration and school coordination • Collaboration with CSBs and wraparound teams ELIGIBILITY REQUIREMENTS (per DMAS): • Must have a DSM-5 mental health or behavioral diagnosis • Must be enrolled in Medicaid or FAMIS • Must have a completed Level of Care (LOC) or CANS assessment • Referral required from a licensed professional, CSB, court, or school NO COST TO CLIENT — Services are billed directly to Medicaid/FAMIS.
